In-Home Support Services
- Individualized Home Support (HIS) with Training: This is meant to tailor in-home support and training to foster independence and enhance daily living skills. IHS, with training, encompasses these supports while also integrating skill development and educational components. This additional training is designed to enhance independence, teaching clients new skills for managing their daily lives. While both services aim to support individuals in their homes, the key difference lies in the element of skill and educational training, with one focusing on immediate assistance and the other on long-term skill development.
- IHS without Training: Personalized care for those needing assistance with daily activities, focusing on safety and comfort. The program mainly focuses on providing essential, non-medical assistance for daily living. This includes aid with personal care, meal preparation, and housekeeping, catering to those who need support in maintaining a comfortable and safe home environment.
- Individualized Home Support (HIS) with Family Training: provides support and training to both the person with a disability and their family. The training is designed to help the family learn how to support the person in their home and community.
Basic Support Services
- 24-Hour Emergency Assistance: Immediate support is available around the clock for emergencies, ensuring help is always at hand. 24-hour emergency assistance service provides on-call counseling, problem-solving, and immediate response for assistance at an individual home due to health or personal emergencies that are not related to an injury or immediate danger.
- Homemaker Services: Assistance with household tasks to maintain a clean, safe, and comfortable living environment. In addition, the program provides non-medical assistance to seniors and adults with disabilities who need help with activities of daily living, such as meal preparation, light housekeeping, and assistance with personal care. It is designed to help individuals remain living in their own homes rather than moving to a nursing home or other facility.
- Night Supervision: Overnight care and monitoring for safety and peace of mind. It is a non-medical service that provides overnight assistance and supervision to individuals with disabilities requiring frequent attention or assistance at night. These services are designed to promote independence, enhance the quality of life, and ensure the safety and well-being of individuals who may need assistance during the night. They can be provided in the individual’s home or in a residential setting.
- RESPITE CARE (In home or out of home): Flexible respite services providing temporary relief for caregivers, ensuring continuous, consistent, and compassionate care to meet the needs of the individual. The service provides temporary relief to primary caregivers, allowing them to recharge and take care of their own needs while ensuring the safety and security of their loved ones.
- Individual Community Living Supports (ICLS): Support focusing on independence and community integration, including assistance with daily living activities and skills development. ICLS is a comprehensive service package that comprises six service components designed to provide older adults with the assistance and support they need to remain in their own homes. These services offer reminders, cues, intermittent/moderate supervision or physical assistance, as required.
